Arterial hypertension remains a leading cause of left ventricular hypertrophy (LVH), diastolic dysfunction, and heart failure with preserved ejection fraction (HFpEF). Conventional echocardiography with standard assessment of left ventricular ejection fraction (LVEF) does not allow the detection of subclinical myocardial damage at the early stages of cardiac remodeling. The introduction of grayscale speckle-tracking echocardiography (STE) has provided new opportunities for the precise assessment of myocardial deformation (strain) and strain rate. This article summarizes current concepts regarding the clinical and hemodynamic factors (blood pressure level, circadian blood pressure profile, arterial stiffness, and myocardial mass) and age-related factors that contribute to early reductions in longitudinal, radial, and circumferential left ventricular deformation in patients with arterial hypertension
